Agree to Club Liability Releases 

I hereby agree to abide by the Glooscap Curling Club’s by-laws and rules and regulations at all times while participating in any activities or events. I acknowledge that curling is an ice sport and there is risk that I may fall while on the ice or elsewhere on club property and injure myself. I further acknowledge that unless I keep my personal belongings with me at all times while I am at the Curling Club there is a risk that they will be lost or stolen.
In making application for activities and events, I accept these risks willingly, and I hereby release the Glooscap Curling Club, Board of Directors, management, volunteers, sponsors and advertisers from any liability for any loss of property or injury or illness incurred while on the ice or while on any part of the premises.
I acknowledge that the Glooscap Curling Club takes photos and videos of events at the club and that these photos and videos may be placed on social media used by the club.  I acknowledge that the personal information collected on this form will be used for the sole purposes of registration for this activity.

I acknowledge that I have read this agreement, that I have executed this agreement voluntarily, and that this agreement is to be binding upon myself, my heirs, executors, administrators and representatives.
